Home and Community Based Services

Active Visions provides service facilitation and personal support through the Adult and Children Home and Community Based Services Waiver Program.  This program aims to provide  individualized, necessary supports, allowing recipients the ability to continue living at home and actively participate in their community.   The goals of this program include increasing independence and empowering individuals to reach their maximum potential through self-determination.

Home and Community Based Services include:

  • Service Facilitation 

  • Personal Support (agency model and/or consumer directed)

  • Developmental Training, Sheltered Workshop, Supported Employment 

  • Behavior Intervention 

  • Adaptive Equipment, Assistive Technology, Home and Vehicle Modifications 

Service Facilitation includes (but is not limited to the following):

  • Home Visits

  • Individual Service Plan (ISP) development and implementation 

  • Assistance with identifying and utilizing approved providers

  • Transportation (ADA. Free Ride, etc.)

  • Food Stamps

  • SSI/SSDI assistance

  • Medicaid/Medicare assistance

  • Guardianship assistance

  • Special Needs Trust referrals

  • Power of Attorney assistance

  • Legal and Advocacy referrals

  • Assistance with other funding sources
